Our mental health forms, letters, and evaluations are completed with care, clinical accuracy, and thoughtful consideration of your individual circumstances.
Documentation requests are based on clinical evaluation, medical necessity, and professional judgment. An appointment may be required prior to completion.
Services may include:
• Work, school, or housing verification letters
• Disability and leave of absence documentation
• School and academic accommodations letters
• Workplace accommodation letters
• Emotional Support Animal (ESA) evaluations and letters
• FMLA, EDD and other disability forms
• Psychiatric treatment or medication verification letters

We do not provide court ordered evaluations, legal assessments, or documentation for active legal cases.

Turnaround time for documentation is up to 7 to 10 business days, with expedited processing available within 1 to 3 business days for an additional fee. Administrative services are not covered by insurance, and payment is required prior to completion. All documentation is completed based on clinical appropriateness.
